Sales terms and warranty for DISH
Network systems
30 Day Satisfaction Guarantee
If for some unlikely reason you are not completely satisfied with a
merchandise purchase, you may return the merchandise prepaid for a refund within 30 days
of shipment. Shipping & handling fees and DISH NETWORK service fees, if applicable,
are not refundable. Returned merchandise must include all original components, literature
and packaging in the same salable condition you received it in. There will be a 15%
restocking fee. Damaged merchandise, cosmetically or otherwise, is subject to a restocking
fee on a case-by-case basis (please call for details). Returned DISH NETWORK receivers
will incur a $35.00 access card replacement fee if they have been activated. Note that it
IS NOT necessary to activate a DISH NETWORK access card to determine DISH NETWORK signal
strength. Thirty (30) days after the shipment date, the purchase price of merchandise is
non-refundable. For instructions on returning merchandise, you must call (310) 473-9846
for a Return Merchandise Authorization number.
Terms of Sale for DISH NETWORK Satellite Systems and
Receivers
The following sales terms allow us (World Wide Satellite) to offer
you deeply discounted retail prices on DISH NETWORK satellite equipment. We are able to do
this through a DISH NETWORK national retailer subsidy program where we are reimbursed only
if certain conditions are met. If you currently subscribe to DISH NETWORK services, or
have in the past, please call us for pricing.
By submitting acceptance to the following sales terms electronically
or by fax, you agree that: |

I (or the DISH NETWORK subscriber
I am purchasing for) am a first-time DISH NETWORK subscriber, residing in a residential,
non-commercial dwelling within the Continental United States and are at least 18 years of
age. |
| 2. |
I (or the DISH NETWORK subscriber
I am purchasing for) will maintain a subscription of DISH NETWORK Top
50 (24.99/month) or
better for 12 consecutive months on each DISH NETWORK Receiver purchased from World Wide
Satellite. [Note: In a multi-Receiver set-up, only one receiver is charged the full
programming rate and all additional receivers are $5.00 per month*.] |

I (or the DISH NETWORK subscriber
I am purchasing for) will activate DISH NETWORK services on each DISH NETWORK Receiver
purchased from World Wide Satellite within 15 days of receiving the equipment, without
exception. |

I (or the DISH NETWORK subscriber
I am purchasing for) will activate DISH NETWORK services using the specific DISH NETWORK
account number provided by World Wide Satellite. This account number will be sent to you
after purchase and can also be obtained by calling World Wide Satellite at 310-473-9846. |

I (or the DISH NETWORK subscriber
I am purchasing for) will activate the DISH NETWORK Primary Receiver before any other DISH
NETWORK Receivers purchased from World Wide Satellite or any other source (the DISH
NETWORK Primary Receiver is one packaged and shipped with the satellite dish). |

I (or the DISH NETWORK subscriber
I am purchasing for) will use the DISH NETWORK programming access cards only in the
Receivers they have been paired with at the time we receive them from World Wide
Satellite. |

If I (or the DISH NETWORK
subscriber I am purchasing for) fail to fulfill the terms listed above, I am
pre-authorizing World Wide Satellite to charge the credit card I used to purchase the
merchandise with the following fees:
- A one-time early cancellation / non activation fee of $250.00.
A one-time early cancellation / non activation fee of
$125.00 for each additional DISH NETWORK Receiver (applicable only in multi-receiver
purchases).

| By submitting acceptance to these sales terms
electronically or by fax, I as the purchaser also understand and agree to pay World Wide
Satellite any and all fees associated with the collection of the above fees, including
those fee amounts, should the credit card I originally used have insufficient funds or be
declined for any reason whatsoever. If my original purchasing credit card is declined for
any reason I agree to submit a currently valid credit card for the required 12 month
period from purchasing date. That's all there is to it. Simply
call (310) 473-9846 and we will fulfill your order and set-up your DISH NETWORK account
with your programming of choice.

* Additional Receiver rates may vary in NRTC areas. Some NRTC areas
may have an initial activation fee.
